We don't know enough about you over the internet to tell you exactly what is wrong. Don't be embarrassed to see a doctor, they see way worse things, trust me. Haemorrhoids (piles) don't usually appear in the anal cavity. You may have polyps which are easily treated... by a doctor. If you are getting period pain around your anus, and it's just started, there could be something really wrong. Please, see your doctor.
